They work by inhibiting the action of a certain enzyme in the male body which, when in large concentration, does not allow the smooth muscles in the penis to relax. As soon as the enzyme is inhibited by an ED drug, the smooth muscles relax, which results in an increased blood flow to the penis. So as you can see, PDE5 inhibitors act on the physiological level.

Many patients wonder if they can get an erection any time after taking these drugs. The truth is, taking an ED pill will not lead to an automatic erection. One has to be stimulated sexually for the drug to start working; they are not aphrodisiacs.
